Rehab that follows your protocol from the clinic to the gym floor
Adapt's registered physiotherapists assess your patient, work within the protocol you send, and can carry the rehab onto a supervised gym floor in North Sydney when the plan calls for it. Send a referral through our referral page, or your patient can book directly, since no referral is needed for a physiotherapy appointment.

What to send and how it is used
The more your patient's physiotherapist knows before the first appointment, the closer the plan can follow your guidance.
| What to include | How Adapt uses it | |
|---|---|---|
| Protocol | The post-operative protocol or rehab guidelines | Each stage of the plan is set within it. |
| Restrictions | Weight-bearing, range or loading limits | Built into the program until the protocol allows more. |
| Imaging and reports | Scan reports or operation notes | Read at the assessment with the physical examination. |
| Relevant history | Other conditions or medicines that affect exercise | Used to set the starting load and what to watch. |
| Goal | The work, sport or daily task ahead | Shapes the later stages of the program. |

Rehab that continues between appointments
When gym-based physiotherapy suits your patient, their program moves onto the gym floor in supervised sessions run from the physiotherapist's plan, and anything that needs a clinical look goes straight back to the physiotherapist.
How it worksHow a referral works
Send details through the referral page, and your patient can book a physio or a gym-based physio assessment at any time.
Send the details
Send the referral through the form or by email, and attach the protocol or rehab guidelines you want followed.
Book the assessment
Your patient books a physiotherapy assessment online or by phone, because a referral is not required to start.
Assess and plan
The physiotherapist assesses your patient within your protocol, then recommends appointments or gym-based physiotherapy from what they find.
Review as it progresses
Reviews are set from that assessment, with strength testing added when a measurement will inform the next step.
Rehab we plan after surgery and injury
These are the conditions we most often plan from a referral, each within the protocol you send.
ACL reconstruction
The plan stays inside the surgical protocol, with strength and hop testing used at reviews to decide the next block.
Knee replacement
Movement and strength work is staged from the protocol you send, then reviewed as the knee tolerates more load.
Shoulder rehab
Early movement and later loading follow the repair protocol, then the tasks your patient needs at work or in sport.
Back pain and sciatica
The physiotherapist assesses the presentation, including sciatica, and sets what to keep doing while load is progressed.
Other surgery
Early appointments follow your protocol, and supervised gym sessions start only when that protocol allows it.
The physiotherapists who see your patients
Adapt's registered physiotherapists, Sam Baylis and John MacAuliffe, see referred patients and plan within the protocol you send.

Does my patient need a referral or a care plan?
No, your patient does not need a referral to book physiotherapy at Adapt, and appointments are private. If they have a GP care plan or another funding arrangement, they should tell the team before booking so we can confirm how it applies. Current fees are confirmed before they start.
Fees and rebatesStrength testing when a number will help
When a measurement will inform the next decision, strength testing with VALD ForceDecks and ForceFrame compares sides and change over time. Your patient's physiotherapist reads those numbers at review, and James Bowen, Director and Head of Training, leads the supervised sessions that follow the updated plan.
How we test

Questions from referrers
Refer a patientCan I send a specific post-operative protocol?
Yes, attach the post-operative protocol or rehab guidelines to an email, or send them with the referral, and the physiotherapist plans each stage within those limits. Restrictions on weight-bearing, range or loading stay in the program until the protocol allows more.
Can I refer to a particular physiotherapist?
Yes, name them in the referral, and your patient can book with that physiotherapist subject to availability. Sam Baylis and John MacAuliffe are the registered physiotherapists who see referred patients at Adapt. If that diary has no free times, the team will say so before a booking is made.
At what stage after surgery can I refer?
You can refer at any stage after surgery. Early appointments follow your protocol, and supervised gym sessions start when that protocol and the physiotherapist allow it. See rehab after surgery for how the plan is staged around those limits.
What will rehab cost my patient?
It depends on the mix of appointments in the plan. Physiotherapy consults may be claimable on extras cover that includes physio, while supervised gym sessions and strength testing are not. The team confirms the current fee before your patient starts. Fees and rebates explains how claiming works at Adapt.
Do you see patients who have not had surgery?
Yes, Adapt physiotherapists also see people who have not had surgery, including injuries and long-running musculoskeletal conditions such as back pain or shoulder problems. A referral still helps when you want restrictions followed, and your patient can book physiotherapy directly.
Who do I contact with a question about a referral?
Call or email the team, or send the question through the referral page. The clinic number is 0417 214 945 and the email is headoffice@adapthealth.io. Replies come from the team by email or phone.
