Refer a patient to Adapt
Send your patient's details and the reason for referral through the form below, and email any protocol or reports to the team. Someone from the team replies to you by email or phone, and your patient can book a physio online at any time, since a referral is not needed for physiotherapy.
Send a referral
Include your name and practice, your patient's name and phone number, the reason for referral and any restrictions. Email protocols and reports to the address beside the form.

What happens after you send it
Your patient books an assessment with one of our registered physiotherapists, Sam Baylis or John MacAuliffe, who plans within your protocol and recommends standard appointments or gym-based physiotherapy from what they find.
Book an assessmentHow should I send scans and operation notes?
Email them to headoffice@adapthealth.io with your patient's name in the subject line, or your patient can bring copies to the first appointment. The physiotherapist reads scans and operation notes with the examination, so either route is enough.
Contact the teamWhere your patient will train
Supervised sessions run on the gym floor from the physiotherapist's plan, with coaching led by James Bowen.

Referral questions
Send a referralCan my patient book before the referral arrives?
Yes. Physiotherapy at Adapt does not need a referral, so your patient can book online or by phone at any time. Sending the referral still helps the physiotherapist plan within your protocol at that first appointment.
What if there is no written protocol?
The physiotherapist plans from the assessment and any surgeon guidance your patient brings. If a protocol arrives later, it can be read at the next review. See post-surgical rehab for how Adapt follows a surgeon's protocol.
Can I refer for gym-based physiotherapy directly?
Yes. Note gym-based physiotherapy in the referral, and the physiotherapist confirms at the assessment whether supervised gym sessions suit the presentation and the protocol. Your patient can also book a gym-based assessment if that is already the intended route.
What will my patient's first appointment be?
The first appointment is a physiotherapy assessment, usually 45 to 60 minutes. The physiotherapist examines your patient, reads any protocol or reports you sent, and sets the next appointments or gym-based sessions from what they find.
How is my patient's information handled?
Referral details are used to arrange your patient's care. Someone from the team replies to you by email or phone using the contact you give on the form. Include the reason, restrictions and protocol with the patient's name and phone number.
